Glasgow City Care Home Nursing Team

The overarching goal of the team is to offer a collaborative, integrated approach to enhance care home services within both older persons and adult care homes within Glasgow City.

Our Practice Development Team carry out regular Care Assurance visits using our Care Homes Assurance Tool (CHAT). The tool utilises a strength-based approach to highlight and celebrate good practice, identify any areas of improvement and as a team, provide support, guidance and education to meet the improvements. The tool can help shape priorities and allow for proactive support, development and training as agreed with our care home managers.

Our Care Home Liaison Nurses are pivotal in supporting nursing home staff. They provide person centred clinical advice, clinical interventions as well as supporting care home nurses with their continual education and training.
